Revision Arthroplasty of the Acetabular Component: Minimum Two-Year Follow-up

Abstract
Purpose: We evaluated the results of osteolysis induced cemented and uncemented acetabular revisions.

Material and Methods: We retropsectively reviewed the clinical and radiological results of 38 hips(34 patients) which were followed longer than 2 years after revision due to osteolysis. Preoperatively, acetabular osteolysis was classified according to the system of Rubash et al. Intraoperatively, the acetabular bone defect was classified again and treated according to the modified AAOS classification.

Results: There were no significant differences in terms of clinical and radiological results between cemented revision total hip arthroplasty. The average period of boony union between acetabular and grafted bone was 9 months, ranging from 7 to 17 monts. The average period for the cemented type was 11 months, ranging from 8 to 25 months, and that for the uncemented type was 7 months, ranging from 5 to 13 months. One re-revision totala hip arthroplasty was performed.

Conclusion: Using the pre-operaive Rubash classificatin and Intra-operative Glassman-AAOS classification as guide lines, acetabular revisions were carried out with cememted and uncemented acetabular cups. The incidence rate of radiolucent line was higher in the cemented types and complete bone union between acetabular and grafted bone was noted to occur earlier in the uncemented type.
